WHAT CAN JOHN SENSE ABOUT YOUR CHARACTER?: First and foremost is, of course, John's relationship to the Eye. It's somewhat different in Kenos, as Oblivion destroyed the main "bodies" of the Fears along with his world prior to his arrival here. The piece of the Eye that remains entangled within his shard is an infinitesimal fraction of it (the part that had become a part of him after his becoming an avatar), dramatically limiting its overall strength and influence. If inspecting the composition of his soul and/or shard, it would be pretty obvious that there is another presence, bizarre and eldritch, within, it would give a strong impression of watching its observer back if scrutinized. Based on what the Boneturner says about his ribs, it seems that the eldritch power of the Eye has also saturated John on a physical level as well. So it probably just seems really weird? (Also speaking of the Boneturner and what's weird, I tend to assume he's still missing those two ribs that were pulled out of him, so that's probably... odd.)
Now, I was always under the impression that most avatars are essentially reanimated after a sort of "death" and then sustained by the power of their Fear, but some stuff in late Season 5 kind of challenged that assumption of mine. Regardless, he was pretty much dead for six months prior to making his decision to return to life as an avatar, so there's also that. I think he would certainly comes off as "alive" in a necromantic sense, though the thalergenic/thanergenic vibes might be super weird.
Also, since the Fears can't really exist without one another, he carries with him much smaller nascent versions of all thirteen of the other Fears, predominantly in the Marks they have left upon him but also in smaller part within the statements that have been inscribed upon him as the Archivist. None of these are really "active" as the Eye currently is, but he is essentially still primed and ready to once again act as the conduit to bring them back into the physical world in some way, shape, or form under very specific and ideal circumstances. Really, when I think about it from the perspective of the world of The Locked Tomb, I feel as though the Emperor would likely be able to recognize that John had been the subject of... something...? There could be some interesting comparisons between necromantic formulae and how rituals work in The Magnus Archives.
